  • Proof of financial responsibility may be submitted in the form of an SR22 Certificate of Insurance completed by the home office of an insurance company and submitted to the Secretary of State before reinstatement of driving privileges or issuance of a restricted driving permit; A Secretary of State reinstatement fee may also apply.

  • The SR22 form may be obtained by contacting individual insurance companies. Standard liability insurance or insurance binders are not acceptable. When payment is made to an insurance agency, the agent will submit a request for an SR22 certificate to the central office of the insurance company. The SR22 Certificate will be sent directly to the Secretary of State. The insurer will receive a copy of the SR22 from the insurance company as well as a letter from the Secretary of State upon acceptance.

  • The SR22 Certificate may be in the form of an Owner’s Certificate, an Operator’s Certificate, or an Owner’s/Operator’s Certificate. The Owner’s Certificate is required for those who own vehicles and wish to obtain license plates for it; an Operator’s Certificate should be filed for those who do not own vehicles.

  • The amount of coverage for an SR22 Certificate must be at least $20,000 for one person killed or injured, $40,000 for two or more people killed or injured, and $15,000 for property damage, and the insurance policy must conform to the provisions of the Illinois Vehicle Code.

  • The SR22 must be maintained for up to three years, depending on the offense. Any lapse in coverage will result in a driver’s license suspension.  To avoid future suspensions, it is recommended that the individual renew their insurance a minimum of 30 days before expiration by calling Flex Insurance at 877-323-FLEX (3539)
